Dog Surrender Form

Please complete this form as honestly and thoroughly as possible — it helps us match your dog with the most suitable new home. It should take around 15–20 minutes to complete if you have the following ready:

  • Basic details about your dog (age, breed, sex, vaccination & sterilisation status, behaviour, etc.)

  • Your reason for surrendering the dog

  • At least 2 clear, high-quality photos of your dog (see Section 5 for what’s acceptable)

  • (Optional) A 10-second video of your dog playing or interacting

🔴 IMPORTANT:
  • If you are giving up more than one dog, please fill in a separate form for each dog.

  • If the dogs are bonded and must stay together, please let us know in Section 4.

  • We cannot make your dog visible for adoption unless the photos are clear, front-facing, and follow the guidelines in Section 5. Blurry, dark, distant, or fence-blocked photos will delay the process and require resubmission.

All details are for office use only and will not be shared publicly.

All Personally Identifiable Information (PII) data is encrypted using AES with a 256-bit site encryption key, storing it encrypted both on disk and at the application level, ensuring data protection and compliance with POPI regulations. 

This form is designed to give your dog the best chance at a happy new home — thank you for taking the time to do it properly.

Please upload at least 2 GOOD quality photos and one short (max 10 sec) video.
IMPORTANT: Good photos greatly improve your dog’s chances of finding a loving home!
  • Take a clear front-facing photo at the dog’s level (not from above).

  • Avoid photos through gates, fences, or while the dog is sleeping.

  • Avoid blurry, dark, or distant photos.

  • A happy, alert expression works best.

  • Optional: Include a 10-second video of your dog playing.

⚠️ Please Note:
We cannot publish a dog’s profile on the Adoptable Labs page unless we receive clear, high-quality photos that follow the guidelines above.

If you upload unusable images (e.g. blurry, dark, pixelated/low-res, far away, lying down, not looking at the camera or taken through fences/gates), we will not proceed until suitable photos are provided.

Let’s not waste your time or ours — a decent photo could mean the difference between a dog waiting weeks vs. days for a loving home. It’s really not that hard — please take a moment to do it properly.
